1) Therapist vs. Counselor vs. Social Worker: What’s the Difference?

Let’s clear up the biggest source of confusion first.

“Therapist” is commonly used as a broad description for a professional who provides psychotherapy. It isn’t one universal license with one universal educational path. Depending on the state and setting, a therapist may be a:

• Licensed Clinical Social Worker (LCSW)
• Licensed Independent Clinical Social Worker (LICSW)
• Licensed Professional Counselor (LPC)
• Licensed Mental Health Counselor (LMHC)
• Licensed Clinical Professional Counselor (LCPC)
• Licensed Professional Clinical Counselor (LPCC)
• Licensed Marriage and Family Therapist (LMFT)
• Psychologist
• Other independently licensed behavioral health professional

So, when someone says, “I want to become a therapist,” the next question is really: Which professional route do you want to take to become one? Social Work and professional counseling are two of the most common routes.

A Clinical Social Worker receives graduate education in Social Work, including clinical practice alongside broader training in systems, policy, human behavior, advocacy, communities, organizations, and social environments. Professional counselors generally receive graduate education centered more specifically on counseling theory, clinical assessment, human development, counseling skills, diagnosis, treatment, and therapeutic practice.

Both paths can lead to careers providing psychotherapy.

That’s why choosing between them is less about asking, “Which profession gets to do therapy?” and more about asking, “Which educational philosophy and range of career options fit me best?”

If you’re already leaning toward professional counseling, the Agents of Change guide to How to Become a Licensed Mental Health Counselor (LMHC/LPC) provides a much deeper walkthrough of that specific route.

2) How to Become a Therapist, Counselor, or Social Worker: Career Paths in Mental Health at a Glance

Although licensing rules vary substantially from state to state, the three paths can be compared broadly.

Career Path	Typical Graduate Degree	Common Licenses	Common Work
Clinical Social Work	MSW	LMSW, LCSW, LICSW, LISW and state variations	Therapy, hospitals, schools, community mental health, case management, healthcare, nonprofits, private practice
Mental Health Counseling	Master’s in Counseling or Clinical Mental Health Counseling	LPC, LMHC, LCPC, LPCC and state variations	Individual therapy, group therapy, community mental health, hospitals, treatment programs, private practice
Marriage and Family Therapy	Master’s in Marriage and Family Therapy or qualifying related program	LMFT and state variations	Couples therapy, family therapy, relationship concerns, individual therapy
Addiction Counseling	Varies considerably by state and credential	CAC, LAC and numerous state-specific credentials	Substance use treatment, recovery programs, residential treatment, outpatient care
Psychology	Doctoral degree for psychologist licensure in most cases	PhD/PsyD plus psychologist license	Therapy, assessment, psychological testing, research, consultation

The table makes the choices look tidy. Careers rarely stay that tidy.

A Clinical Social Worker could spend five years in a children’s hospital, move into outpatient psychotherapy, become a clinical supervisor, and later open a private practice. A Professional Counselor could begin at a community mental health agency, specialize in trauma, transition into a hospital system, and eventually provide telehealth independently.

Your first job does not have to become your permanent professional identity.

That’s one reason to think about the flexibility of the license itself, not simply the first setting in which you want to work.

How to Become a Social Worker

For someone searching how to become a Social Worker, the first distinction to understand is that Social Work includes far more than psychotherapy.

Social Workers work in mental health clinics, schools, hospitals, child welfare agencies, government programs, hospice organizations, substance use programs, universities, nonprofits, community organizations, policy settings, veterans’ services, private practices, and many other environments.

The Bureau of Labor Statistics counted more than 816,000 Social Worker jobs in 2025, including large groups working in child, family, and school Social Work, healthcare Social Work, and mental health and substance use Social Work. Overall Social Work employment is projected to grow 6 percent from 2025 through 2035, with mental health and substance abuse Social Work projected to grow even faster.

Step 1: Decide Whether You Need a BSW, MSW, or Both

A Bachelor of Social Work, or BSW, can qualify graduates for many generalist Social Work roles and, depending on the state, certain Social Work licenses.

If your goal is clinical practice or independent psychotherapy, however, you’ll generally need a Master of Social Work, or MSW.

You do not necessarily need a BSW before entering an MSW program. Students regularly enter MSW programs after undergraduate degrees in psychology, sociology, education, public health, human services, and plenty of other disciplines.

There is one major advantage to earning a BSW first: advanced standing.

Students who graduate from qualifying accredited BSW programs may be eligible for advanced-standing MSW programs, allowing them to complete the graduate degree more quickly than students entering from another undergraduate major.

If you’re comparing programs, review Online MSW Programs: How to Choose an Accredited Master’s in Social Work before deciding based solely on price, convenience, or program length.

Step 2: Check Social Work Accreditation

This is where prospective students need to slow down.

The Council on Social Work Education, or CSWE, is the specialized accreditor for Social Work education programs in the United States and its territories. Accreditation can affect eligibility for Social Work employment, examinations, and licensure, and CSWE notes that most state and territory licensing requirements stipulate graduation from a CSWE-accredited program for Social Workers pursuing licensure or a licensing exam.

That means a program being called a “social services master’s” or including the word “Social Work” somewhere in its description does not automatically mean it prepares you for the license you want.

Before enrolling, ask:

1. Is the exact program accredited by CSWE?
2. Does the program meet educational requirements in the state where I expect to become licensed?
3. How are field placements arranged?
4. Does the curriculum prepare students for the type of Social Work I want to practice?
5. If I move states later, how portable will this education be?

That last question matters more than many graduate students realize.

Step 3: Complete Field Education

Social Work education includes supervised field experience, giving students the opportunity to apply classroom learning in real service environments.

Placements vary widely. One student may intern in a psychiatric hospital while another works in child welfare, a school, a nonprofit, or an outpatient therapy program.

Try to select placements strategically when possible.

If you think you want to become a therapist, a clinically oriented placement can give you exposure to assessment, treatment planning, documentation, interdisciplinary care, and direct client work.

If you’re more interested in schools, read Social Work in Education Settings to see how Social Workers support students, families, school systems, and educational access.

Step 4: Understand Your State’s Social Work Licenses

Here’s where things become alphabet soup.

Depending on your state, you may encounter credentials such as:

• LBSW
• LSW
• LMSW
• LGSW
• CSW
• LCSW
• LICSW
• LISW
• LISW-CP
• LCSW-C

Unfortunately, identical acronyms don’t always carry identical meanings across states.

The guide to Social Worker Licensing Acronyms explains how degrees and professional licenses differ and why you shouldn’t assume that an LSW, LMSW, or LCSW means exactly the same thing everywhere.

Even more useful when you’re planning your career is the updated Social Work Licensing Requirements by State guide, which compares licensing structures across all 50 states and Washington, DC.

State requirements can differ in examination requirements, supervised hours, eligible supervisors, background checks, jurisprudence examinations, clinical coursework, associate credentials, and when you’re permitted to begin accumulating supervised experience.

Check the board before you start collecting hours. Seriously. Finding out two years later that your supervision didn’t qualify is the kind of career surprise nobody wants.

Step 5: Complete Any Required Licensing Exam

Many Social Work licenses require an Association of Social Work Boards, or ASWB, examination, although requirements vary by jurisdiction and license.

The exact exam you need depends on the license you are pursuing and your jurisdiction. Don’t assume that graduating with an MSW automatically tells you which ASWB exam to register for.

Agents of Change maintains an extensive ASWB Exam FAQ and a separate guide to registering for the ASWB exam for candidates reaching this stage of the process.

Step 6: Complete Supervised Clinical Experience if You Want Independent Practice

If your goal is becoming a Clinical Social Worker who independently provides psychotherapy, completing the MSW is usually the beginning of your clinical licensure process rather than the end.

Most jurisdictions require a period of supervised post-graduate clinical practice before independent clinical licensure.

The number of required hours, supervision rules, and acceptable practice activities vary significantly.

After reaching full licensure, the possibilities become broader. The Agents of Change guide to career options with a BSW, LMSW, or LCSW explores how different Social Work credentials can lead into clinical practice, healthcare, children and families, substance use treatment, education, community practice, leadership, and other roles.

There’s also a useful next-stage resource for new licensees: I Passed the ASWB, Now What? A Post-Licensure Roadmap for New LMSWs and LCSWs.

How to Become a Mental Health Counselor

What if you’re less interested in the broader Social Work profession and know from the beginning that counseling is where you want to build your career?

Then professional counseling may be the more direct fit.

Mental Health Counselors provide services such as assessment, treatment planning, individual counseling, group counseling, psychoeducation, crisis support, and other behavioral health interventions within their legal scope of practice.

According to the Bureau of Labor Statistics, mental health counselors typically need a master’s degree and internship experience. Employment for substance abuse, behavioral disorder, and Mental Health Counselors is projected to grow 18 percent from 2025 through 2035, substantially faster than the average for all occupations.

Step 1: Earn a Bachelor’s Degree

Your undergraduate major doesn’t necessarily have to be counseling.

Psychology, human services, sociology, Social Work, education, behavioral science, and related majors are common starting points.

What’s more important is preparing yourself for graduate admission and understanding prerequisites before applying.

Step 2: Earn a Qualifying Master’s Degree

Independent professional counseling generally requires graduate education.

Program titles can include:

• Clinical Mental Health Counseling
• Counseling
• Professional Counseling
• Counselor Education
• Other state-approved counseling degrees

Accreditation deserves careful attention here too.

The Council for Accreditation of Counseling and Related Educational Programs, better known as CACREP, accredits graduate counseling programs and specialized counseling practice areas. As of 2026, CACREP reports accrediting more than 969 master’s and doctoral programs at hundreds of institutions.

Your state’s board determines the educational requirements for licensure, so don’t assume that every counseling master’s degree leads to every counseling license.

The deeper Agents of Change resource on licensing requirements for counselor careers can help you understand how graduate education, examinations, supervised practice, and licensing fit together.

Step 3: Complete Practicum and Internship Training

Graduate counseling programs include supervised clinical training.

This is where you’ll begin moving beyond textbook knowledge and practice skills such as:

• Building therapeutic relationships
• Conducting assessments
• Setting treatment goals
• Applying counseling theories
• Documenting services
• Handling ethical questions
• Receiving and using supervision
• Working with clients from different backgrounds and communities

It can be humbling. That’s part of the point.

Graduate training isn’t supposed to produce a professional who never needs consultation. It’s supposed to help you recognize what competent practice requires, including when you need supervision, collaboration, or additional education.

Step 4: Apply for the Appropriate Associate or Provisional Credential

Many states have a supervised pre-independent stage.

Titles vary considerably. You might encounter credentials indicating an associate, temporary, limited, intern, or provisional status.

Your state board will determine whether this credential is required, when you can apply, and what activities you can perform while working toward full licensure.

Step 5: Complete Required Supervised Experience

Much like Clinical Social Work, professional counseling typically requires supervised post-master’s practice before independent licensure.

Again, state rules matter.

A board may regulate:

• Total experience hours
• Direct counseling hours
• Supervision frequency
• Supervisor credentials
• Group versus individual supervision
• Approved work settings
• Documentation
• Time limits for completing experience

Never build your plan around the requirements your classmate heard from someone licensed in another state.

Step 6: Pass the Required Examination

States may use examinations such as the National Counselor Examination, or NCE, the National Clinical Mental Health Counseling Examination, or NCMHCE, or another approved examination depending on the jurisdiction and credential.

Because requirements change, your licensing board should be the final authority.

For a full walkthrough, see How to Become a Licensed Mental Health Counselor (LMHC/LPC).

If your state specifically uses the LCPC title, Agents of Change also has a separate guide to becoming a Licensed Clinical Professional Counselor (LCPC).

What Are the Different Types of Counselors?

Searching “types of counselors” can produce an overwhelming list because counseling careers are often organized by population, setting, specialty, or credential.

Here are several of the most common paths.

Mental Health Counselor

Mental Health Counselors work with clients experiencing concerns such as anxiety, depression, trauma, adjustment difficulties, stress, relationships, and other behavioral health needs.

They can work in outpatient clinics, community agencies, hospitals, residential programs, private practices, integrated care settings, and telehealth.

Addiction Counselor

Addiction counselors specialize in substance use disorders and recovery.

Depending on the jurisdiction and position, the path may involve a general behavioral health license, specialized addiction certification or licensure, or both.

If this specialty interests you, compare the Agents of Change guides to How to Become an Addiction Counselor and How to Become a Substance Abuse Counselor.

School Counselor

School counselors typically work within educational environments and help students with academic, social, emotional, and career development.

This is a distinct professional pathway from school Social Work, even though professionals in both fields frequently collaborate.

If schools are where you picture yourself, research your state’s certification or credentialing requirements carefully because education systems can impose requirements beyond a general behavioral health license.

Marriage and Family Therapist

Marriage and Family Therapists receive specialized preparation in relational and family systems.

They’re widely associated with couples and family therapy, although LMFTs may also provide individual psychotherapy within their scope of practice.

The U.S. Bureau of Labor Statistics projects employment of Marriage and Family Therapists to grow 13 percent from 2024 through 2034.

Rehabilitation Counselor

Rehabilitation counselors help individuals navigate disabilities, health conditions, employment, independence, adjustment, and participation in community life.

They may work within vocational rehabilitation agencies, healthcare programs, community organizations, schools, or private services.

Career Counselor

Career counselors help people explore education, employment, strengths, interests, career changes, and occupational goals.

Their work may happen in schools, universities, workforce programs, private practices, or organizational settings.

Sex Therapist

Sex therapy usually isn’t the first professional license someone obtains.

Instead, professionals often become licensed through a qualifying mental health discipline, such as Clinical Social Work, counseling, psychology, or marriage and family therapy, and then pursue specialized education and training.

Agents of Change’s The Journey to Becoming a Sex Therapist explains this specialization in greater depth.

Can Social Workers Become Therapists?

Absolutely.

This is one of the most persistent misunderstandings among people researching mental health careers.

Clinical Social Workers are among the major providers of psychotherapy in the United States. Their training differs from counseling training, but appropriately licensed Clinical Social Workers can assess and treat mental health conditions and provide psychotherapy within the scope of their jurisdiction and competency.

Social Work may be particularly attractive if you’re interested in therapy but also want the flexibility to work in systems beyond traditional outpatient counseling.

For example, Social Workers may move among:

• Private practice
• Hospitals
• Schools
• Child welfare
• Community mental health
• Hospice
• Substance use treatment
• Crisis services
• Veterans’ programs
• Advocacy
• Administration
• Policy
• Program development
• Supervision
• Higher education

The profession continues to expand into areas such as integrated healthcare, teletherapy, crisis intervention, technology, and interdisciplinary care. The Agents of Change resource on Emerging Roles for Clinical Social Workers offers a closer look at some of these directions.

Should I Become a Social Worker or Mental Health Counselor?

This might be the most useful question in the entire article.

There is no universally better profession.

There is a profession that may be a better match for you.

Social Work May Be a Strong Fit If…

You may prefer Social Work if you:

• Want clinical therapy to remain one of several possible career directions.
• Are interested in how families, communities, institutions, inequality, policy, and environments affect mental health.
• Want the option to move between clinical and nonclinical work.
• Are interested in hospitals, schools, child welfare, government, nonprofits, healthcare, advocacy, or community programs.
• Like thinking about both individual problems and larger systems.
• Want an MSW that could support a broad range of professional pivots.

Social Work can be especially appealing for someone who thinks, “I want to become a therapist, but I don’t know whether I’ll want to provide therapy forever.”

Counseling May Be a Strong Fit If…

Professional counseling may be appealing if you:

• Know that psychotherapy and counseling are your central career interests.
• Want graduate education heavily focused on counseling practice.
• Are especially interested in counseling theories, interventions, diagnosis, treatment planning, and therapeutic skills.
• See yourself working in outpatient mental health, private practice, community mental health, or other counseling-intensive environments.
• Identify strongly with the Professional Counselor role.

If you’re trying to picture what counseling looks like across settings, compare the guides to becoming a licensed counselor in community mental health and a licensed counselor in a hospital.

What If Private Practice Is My Goal?

Both Clinical Social Workers and independently licensed Professional Counselors can operate private practices when permitted by their state licenses.

So you don’t have to choose Social Work simply because you’ve seen LCSWs in private practice, nor do you have to choose counseling because you associate the word counselor with therapy.

Instead, work backward.

Ask:

1. Which independent clinical licenses are recognized in my state?
2. Which graduate programs qualify me for them?
3. How long will supervised practice take?
4. What populations do I want to serve?
5. Do I value the broader Social Work curriculum or the counseling-focused curriculum more?
6. What happens if I decide private practice isn’t right for me?

You can also read Becoming a Licensed Professional Counselor in Private Practice for a closer look at the counseling route.

How Long Does It Take to Become a Therapist?

There’s no single answer because “therapist” doesn’t represent one licensing pathway.

For many master’s-level professionals, a rough timeline may look something like this:

1. Bachelor’s degree: approximately four years for a traditional full-time program.
2. Master’s degree: commonly one to three years depending on the profession, program structure, previous education, and enrollment status.
3. Post-graduate supervised practice: often several years, depending on jurisdictional requirements and how quickly qualifying hours accumulate.
4. Examination and licensing process: timing varies by state and profession.

Someone entering an advanced-standing MSW program after earning a BSW could have a different timeline from someone entering a traditional MSW program with an unrelated undergraduate degree.

A counseling student attending part time while working could have another timeline entirely.

Instead of asking only, “What’s the fastest program?”, ask, “What’s the fastest legitimate route to the license I actually want?”

Those are very different questions.

Does the State Where I Live Matter?

Enormously.

Mental health professions in the United States are regulated largely at the state level.

That means your state’s laws and licensing board may determine:

• What your license is called
• Which degree qualifies
• Whether specific accreditation is required
• Which courses must appear on your transcript
• Which examination you take
• How many supervised hours you need
• Who can supervise you
• Whether you need an associate license first
• Whether jurisprudence or state law examinations are required
• Whether additional trainings are mandatory
• How frequently your license must be renewed
• Which continuing education credits count

This is why generic internet advice about becoming an LCSW, LPC, LMHC, or LCPC can only take you so far.

If you’re researching how to become a Social Worker in California, for example, California’s pathway has important differences from many other states. Start with the current Agents of Change guide to How to Become a Social Worker in California.

Agents of Change also has detailed Clinical Social Work resources for states including Texas, New York, Florida, and Pennsylvania.

For a broader starting point, use the Social Work Licensing Requirements by State guide rather than assuming your state’s rules resemble those of a neighboring jurisdiction.
